INSTRUCTIONS

Application for Admission
Please read ALL instructions carefully.
Incomplete applications will not be processed.

• Complete ALL sections of this form in block CAPITAL letters using a blue or a black ink pen if applying via a hard copy application form.

• Your elective MUST be selected.

• Sign the applicant's declaration on the last page of this form.
  Attach copies of ALL relevant documents to the application form:

  •       * Passport size photograph
          * Birth certificate
          * Passport Bio Data Page
          * School Transcript
          * Academic certificates if applicable (e.g. CSEC, CAPE)
          * Three (3) Recommendation forms (in a sealed envelope with the flap signed, if                               submitting a hard copy application form)​​​​​

  • One recommendation form must be from your English teacher, one must be from your Mathematics teacher, and one must be from the teacher of the area of specialization for which you are applying!

• Pay Application Fee of US$50.
  Applications will not be accepted without payment of application fee.
  Application fee is non-refundable.

  • Cupper Drive #2, LB SCOTT ROAD Note that no payment will be taken at the school. When making payments at the bank kindly make sure that the name of the applicant and CAPE application fee are clearly stated on the receipt. Upload a copy of the receipt if  filling out your application digitally.
